new
patients
You must schedule your first two appointments at
Back to Life Center. Your first appointment you will be a private
consultation with Dr. Lewine. Please bring with you the completed
personal history questionnaire and signed consent form which have
either been mailed to you, or downloaded from our website. You will
review the relevance of your health history to your current situation,
discuss your goals for care, perform a full examination, check your
spinal patterns and have one entrainment session. We will discuss
your response and schedule your follow up appointment. Plan to be
in the office approximately one hour. In the second visit, you will
receive your second entrainment and we will thoroughly discuss your
plan of care. This visit also lasts just about an hour. During your
second visit you will be given a calendar for the month including
office hours. Our intent is to provide you with all of the facts
you need, so that you can make an educated decision about whether
our care is right for you. This gives both you and Dr. Lewine an
opportunity to establish a foundation for successful future of care.
Please
complete these forms and bring them with you to your appointment:

After your first two consultations,
your visits to the office will be a bit different. Firstly, you
need no appointment to come in. Just check the calendar for open
office hours and stop in the office when it is convenient in your
day. When you get to the office, please sign in. Make yourself comfortable
and when there is an available table in the entraining room, come
right in. Entrainments will be in the group room with up to three
other people. Your personal privacy is assured and it is more effective
to entrain in community.
